QUESTION: Would anyone mind sharing their Employee Performance Evaluation forms and also if you have experienced positive feedback from CARF surveyors regarding your form? This is the Evaluation Form we are using. Each departments create their own performance goals which accounts for 75% of the employee's overall score. Soft skills are used by all and account for the remaining 25% of the overall score. We have several different forms, depending on the position. However each position's evaluation form consists of a section where they are evaluated on their job responsibilities and functions. These are pulled directly from their job description's list of essential duties. Each form also has a section where the position is evaluated on a pre-determined list of core competencies such as communication, collaboration, conflict resolution, technical skills, process improvement, etc. We received really great, positive feedback from CARF on our evaluation forms. This is the DSP Evaluation Form we are using. These are new, so I have not yet heard any feedback from surveyors.
